THE ROLE:
To be part of the primary healthcare team providing high-quality and progressive nursing care to meet the holistic healthcare needs of patients. To work to agreed practice standards and protocols. To carry out assigned duties/procedures and assist in the implementation and evaluation of nursing care under the supervision and direction of a Registered Nurse.

Key Responsibilities:
1. To undertake agreed procedures including: glucose tolerance tests, blood pressure checks, height, weight, body mass index, urinalysis, new patient health checks, assisting in minor surgery and supporting patients who are anxious or distressed.
2. To participate in programmes for the management of long-term conditions through relevant clinics.
3. To refer to the primary healthcare team, as appropriate.
4. To work within all relevant Hospital Practice Policies and procedural guidelines, e.g. Infection Control and Chaperoning.
5. To contribute to the development, monitoring and evaluation of healthcare care delivery by the primary healthcare team.
6. To promote effective communication and relationships within the team.
7. To work collaboratively as a team member to achieve the objectives of holistic patient care.
8. To partake in clinical supervision for own development.
9. To actively participate in clinical and practice meetings.
10. To develop own knowledge and practice to meet objectives/changes in service.
11. To make effective use of IT systems by ensuring the consistent and accurate entering of data.
12. To contribute to a satisfactory system of call and recall of patients.
13. To make recommendations to raise standards of practice and service delivery.
14. To contribute to practice/team audit/research activity, as required.
Standard of Care:
1. To participate in the development of practice guidelines within the Hospital.
2. To participate in research activities within the clinical setting.
3. To promote and implement evidenced based practices within the Hospital as appropriate and relevant.
4. To be aware of the patient as an individual and adopt a holistic approach in the delivery of care.
5. To risk assess all patients and carry out manual handling as per Hospital policy.
Communication:
1. To communicate effectively within the multidisciplinary team throughout the Hospital.
2. To co-ordinate patient care and liaise effectively with other disciplines involved.
3. To maintain patient confidentiality at all times.
4. To liaise and communicate effectively with patients and relatives regarding their care and treatment.
5. To adhere to Kingsbridge Policy guidelines for 'Documentation and Record Keeping'.
Health & Safety:
1. To be aware of the Hospital policies and practice in accordance with regulations.
2. To attend annual statutory training in fire, manual handling, infection control and Cardio-pulmonary Resuscitation.
3. To report to the Ward Manager any reportable incident, or accidents involving staff, patients or visitors.
4. To report any defective or faulty equipment to the Ward Manager or Nurse in charge as appropriate.
5. To ensure proper care and usage of equipment in the Ward.
Education:
1. To assist with and participate in the in-service training programme (included staff orientation induction programme) to enhance patient care.
2. To maintain and up-date own professional development.
3. To report on study days/courses attended to other members of the ward.
4. To rotate onto various shifts (as required) for professional update, to ensure statutory and personnel development is achieved in line with the Personal Development Plan agreed during the annual appraisal.
5. Undertake key clinical responsibilities under the supervision of the Ward Manager. These may include: Haemovigilance, taking on link Nurse responsibilities, and specific clinical developments on a project by project basis.

Person Specification:
- 3 GCSE grades A-C including Maths and English or Equivalent.
- At least 2 recent years' paid experience in similar role in clinical setting.
- Demonstrates an understanding of the role of the healthcare assistant.
- Willing to develop clinical skills and competency.
- Able to make judgments where there is more than a straightforward choice of options, e.g. new patient health checks, urinalysis etc.
- Demonstrates self-awareness, i.e. strengths and weaknesses, personal qualities and skills.
- Able to communicate routine information that requires tact and persuasive skills, or where there may be barriers to understanding.
- Standard keyboard skills
- Competency in carrying out clinical observations.
